Each time this happens, I am told the only option is to refax, with no alternative solutions. Given how time-sensitive fertility treatment is, this has been extremely stressful as I prep for a FET . I explained to the front desk representative, Joy, that while I understand their policy requires fax, HIPAA does allow for secure, encrypted provider-to-provider communication. Due to repeated fax delays and failures, I asked if there was any flexibility to accept secure email directly from my provider. I also specifically requested to speak with a manager. Instead, I was directed to Rachel A. from administration leave, who I was later told was not a manager, per my request to Joy. I was also informed that the actual manager, Shauna, would not be returning my call on 4/1. Rachel immediately stated, before fully hearing my concerns, that “the rules are the rules Joy made it clear the first time,” and refused to consider any alternatives. She also incorrectly stated that secure provider to provider email would not be HIPAA compliant. YIKES, I have a healthcare background which I stated. She doubled down and said my occupation doesn’t matter. Racheal clearly has not been trained as a manager to handle such matters, it was very clear by the manner in which I was spoken to, and the wrong info I was getting. She was more concerned with having her co-workers back and not wanting to hear my concerns for a solution to an operational bottleneck impacting my overall care. What was most concerning was the lack of empathy and professionalism. When I expressed that I am still a patient, even as an outside monitoring patient, Rachel responded by saying it seemed like the term “outside” and not a “patient” was “triggering” me. This is an inappropriate and dismissive comment to make to someone undergoing fertility treatment. I was told she was ending the call as it been 12 min and the rules and results are the same despite how many times and different ways I ask for an accommodations and understanding as I am an outside patient from PA, moved via Military orders, have repeated faxed issues with them, and have time-sensitive treatment with FET (including LETROZOLE). I was repeatedly reminded that outside monitoring is considered a “courtesy,” despite explaining that I am part of a military family and traveling back to my home clinic is not feasible. When policies create barriers in time-sensitive medical situations, there should be some effort to find solutions, not just repeat rigid rules. My spouse also latter called and they said nope, still no fax, re-fax or we can call your home clinic . Calling the home clinic can’t stop a delayed or failed fax, having alternative secure methods would. The orders were sent on a TUESDAY AS A BUFFER FOR A THURSDAY , which is tommorow AND would my third cycle day, Tuesday the orders were faxed and nothing but re-fax re-fax on Wednesday all day, at what point do we address the root of the issue? This has been ongoing since FEB. Overall, this experience has been unnecessarily stressful during an already difficult process. My FET includes letrozole , so timing is EVERYTHING! I wish I had chosen a different outside monitoring provider I HAD NO IDEA THEIR FAX SYSTEM and lack of empathy or critical thinking to improve processes would be such a barrier to care. I asked for a manager, was denied escalation ( As Rachel A advised I would be - SADLY TURNED OUT TO BE TRUE), I was given incorrect HIPAA info ,and was not met with their core values of " leading with Respect, Compassion, and Integrity". I get fax is their policy, but they refuse to get that, the current process has not, is not working for me as a outside patient with a home clinic in a different time zone and unreliable fax. It’s a recipe for disaster and they don’t care yet I’m paying over 1,2000 for monitoring with this level of gross.My husband and I first went to NCFMC in 2020. Because there is a three hour time difference, same day clinical decisions depend on results being faxed and received quickly. In my case, same day transmission was inconsistent and required multiple follow ups. I understand they have a process to follow , and that bloodwork and ultrasounds take time, and that FAXS may fail. HOWEVER, when medical decisions are time sensitive, delays and failures in fax communication add significant stress and can impact medical operations. Please add more options to securely and quickly connect with outside monitoring clinics to account of everything! Weekend appointments present an additional challenge. With the time difference and limited coordination hours, there is very little margin for error if results are not sent promptly. There also does not appear to be a dedicated outside monitoring coordination workflow for East Coast Patients.I mentioned I’m from PA and my clinic is PA at every appointment to anyone who will hear. Regardless , patients are responsible for ensuring orders are received, results are faxed, and both clinics confirm receipt. This places a heavy administrative burden on patients who are already navigating a complex and time sensitive treatment cycle. I also have to coordinate flights back to PA and other travel planning so this could at-least be stress free. The clinical staff were courteous during in person visits for outside monitoring , but the operational process for outside monitoring between clinics in different time zones needs improvement ASAP, particularly around reliable same day result transmission and coordination. Having a horrific experience :( Each visit so far is about $1,200 for my outside treatment, so coordination should be smoother and or adjusted for (provider to provider secure email correspondence) for those with outside monitoring across different time zones. Again I did stress the time zone difference and my home state, my spouse can attest, but the current process is the current process so what can I do ……..Dr. Murray is always courteous and an excellent listener. Assistant Val was incredibly compassionate and understanding, especially knowing I don’t have the best insurance coverage. I truly appreciate how the team handled the labs they ordered—they adjusted the billing codes to help ensure my insurance wouldn’t deny the claims. Val was patient, kind, and genuinely sympathetic to my situation. The clinic went out of their way to work with me and find ways to offset my costs. They are the heartbeat of this process and have made such a difference in my experience. Danielle has been unbelievable — calm, confident, and so comforting during the hardest and scariest parts. From prepping for retrieval to talking me through every step in the OR, she brought a level of compassion and reassurance I didn’t even realize I needed. She’s the kind of nurse who makes you feel safe, seen, and genuinely cared for — not rushed or overlooked. And Kaitlyn, my care coordinator, has been my guide since day one. She’s the one answering questions, coordinating schedules, sending reminders, and helping me make sense of meds, appointments, and next steps. She makes everything feel manageable — even when my brain is spinning from information overload. They refused to process the reimbursement of my $375 stating I can still have an over the phone appointment. Well, why would I want an over the phone appointment when I’m already pregnant? They are still refusing to process my reimbursement. Their lack of common sense and their horrible attitude towards their clients is ridiculous. There are many other clinics in the Sacramento area. My insurance is under a physician resident plan, which doesn’t include fertility coverage except for the initial consultation and/or diagnostic testing such as saline sonograms, ultrasounds..etc. I also receive a limited benefit through Carrot. When I scheduled my appointment, I casually mentioned that I had Carrot benefits to use for IVF—and I truly wish I hadn’t. The billing department immediately told me that because I mentioned Carrot, they could not bill my insurance and I would have to be considered a cash-pay patient at $375 for the consultation. While I agreed with them that I do not have fertility treatment coverage for IVF, I explained that I do have insurance coverage for the initial consult and diagnostics after confirming with my network plan and I specifically requested that they bill my insurance for the initial consult so it could apply toward my deductible. They flatly refused. Their reasoning was that if a patient mentions Carrot, they are “not allowed” to bill insurance—a statement that is simply not true. To verify, I reached out to multiple other clinics (two in Sacramento and three in the Bay Area), and all of them had no problem billing insurance for initial consultation and diagnostics (ultrasound) while still allowing me to save my Carrot benefit for other treatment costs such as egg retrieval and transfer. And to no one’s surprise I had coverage for those consults which my insurance accepted and I paid less than the $375 “cash price” this clinic was insisting upon. Again, I am aware that I do not have fertility coverage, all I wanted this clinic to do was use my insurance for the initial consultation. The rest would have been out of pocket using my carrot benefits which I had no problem with. I also asked the billing staff if they could provide me with the CPT codes they were using, since they kept insisting my consult wasn’t covered. They told me they could not provide that information. I even asked if they could confirm if I actually had Carrot benefits, and they admitted they couldn’t—but still refused to submit a claim to insurance simply because I had mentioned it to the front desk. My fault. I decided to cancel my appointment. Frankly, this was confusing, frustrating, and felt like a huge red flag. Additionally, our treatment ended with my right ovary developing a very large cyst that strangled the ovary and I had emergency surgery, we told the clinic and there was no pleasant reaction to our situation, they did not help us at all but they were not moved by our pain either. Today, after almost two months of the surgery, we are left without a baby that was our goal, with less money due to the investment and with the scars from the surgery. It…After being advised, incorrectly, by another fertility clinic closer to us on how to approach our particular fertility issue, we instead sought fertility treatment at NCFMC. It was the best decision we made! We received care from NCFMC from Fall of 2023 through March 2024. All the doctors at NCFMC are excellent. They are caring, compassionate, knowledgeable, and skilled in their discipline. Special thanks to Dr. Lovely who is amazing! Dr. Sahar is also awesome, and very skilled with egg retrieval. Dr. Generao, who specializes in male fertility issues, is also great! Most of the cycle coordinators are great too, especially Wendy who was/is ours. She is very pleasant, caring and compassionate, experienced, and is a good listener. We also loved how she kept an open mind throughout our journey and took the time to answer our many questions (my husband and I are veterinarians and we are very data-driven/literature-driven people; our fertility issue was also a rather unique/uncommon one), or if she did not know the answer, she would promptly reach out to the doctors who could answer our questions. The only constructive thing we have to share is that the insurance/financial department could bet better with communication and maybe more training of staff. Maybe it's different now, but during the beginning of our fertility journey we were being told things that were either not true or were miscommunicated, or we were left out of the loop on some important things. Had we not been so on top of things, some mistakes could have unnecessarily delayed our treatment or possibly left out some key aspects of our treatment. I had some complications after my egg retrieval that were unrelated, and she took the time to bring me in to make sure everything was good on the reproductive end and was able to settle My nerves. I have to give two special shout outs to my nurses, Alissa and Danielle. Alyssa was my nurse coordinator and the amount of questions she had to put up with from me is impressive lol! She was the perfect person to walk me through my treatment.
