Quick steps
- Open Medical Records in the menu bar.
- (Mobile) Select a facility and tap Continue.
- Verify the facility details.
- Choose how to request records (self-retrieve or generate a request).
- Upload the record once you receive it.
Step-by-step
Step 1: Open Medical Records
Press Medical Records in the menu bar.
Step 2: Select a facility (mobile only)
On mobile, choose a facility at the top of the Medical Records dashboard and tap Continue.
Step 3: Verify facility details
Confirm the facility information. If changes are needed, tap Edit to update the Health Record Release form, then return to Medical Records.
Step 4: Choose the request method
Facilities handle records differently:
- Self-retrieve: If records are available online or in person without a request form, retrieve them and upload in Upload Record.
- Request form needed: Tap Generate Request at the Check Request Method step.
Step 5: Generate the request (if needed)
Tap Generate Request, wait for the form to generate, then choose Send it out.
Step 6: Send the request
Email the form if the facility accepts email requests. If they require fax, use Fax.
Step 7: Upload the record
Once you have the record, upload it in the Upload Record step.
